Job Description

The Rehab/Prospective Payment System (PPS) Referral Coordinator is a professional Registered Nurse who reviews patient referrals to the Rehab Unit, both in-house and from outside facilities. Coordinates referrals to promote timely admission. Assists with data collection for the QI process. Identifies and assists with Rehab specific education needs.

Experience/Qualifications

• Knowledge of patient care standards and practices is normally acquired through completion of a nursing program. Bachelor’s degree in nursing preferred.

• A minimum of two years’ related experience.

• Possesses an active Wisconsin RN nursing license.

• Working knowledge of IRF PAI and FIM.

• FIM (Functional Improvement Measurement) credentialed. PPS certification desired.

• Current knowledge of Rehab CMS rules and regulations.

• Interpersonal skills necessary to effectively communicate with physicians, nursing personnel and team members.
