Licensed Practical Nurse (LPN) Symbii Home Health and Hospice is seeking a Licensed Practical Nurse to join our amazing team! Why choose Symbii Home Health and Hospice? Community involvement – Develop relationships with caregivers, facilities, and physician offices. What will you do as an LPN at Symbii? Provide direct patient care. Implement plan of care initiated by the registered nurse. Document accurately and timely in accordance with the plan of care. Collect assessment data and perform observations to report to and collaborate with RN(s) and physician(s). Coordinate home health services, reporting identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietitian) to the registered nurse and/or Assistant Director of Nursing or Director of Nursing. Participate in personal and professional growth and development. Perform other duties as assigned by the registered nurse. Job Requirements Must possess a valid LPN license in the State of Idaho. Must hold a current CPR certification. Must hold a valid driver’s license and current auto insurance.
