Home Dialysis Nurse: In-Home Hemodialysis & Peritoneal Care

Proforma Health

Proforma Health is seeking an experienced RN to provide training and ongoing support to patients choosing home dialysis modalities, including peritoneal dialysis and hemodialysis. You will conduct home visits to assess environments, document care in electronic medical records, participate in infection control, manage supplies, and collaborate with the healthcare team to monitor patient outcomes and safety. This role requires an active RN license, 12 months of clinical experience with dialysis, #J-18808-Ljbffr Proforma Health
