Wellness Coach - Supportive Housing (Re-Entry)

A nonprofit services organization in New York City is seeking a new Wellness Coach to join their team in Brooklyn, connecting people to necessary community services for recently released individuals.
About the Opportunity:
- Schedule: Monday to Friday
- Hours: 9am to 5pm
- Setting: Supportive housing program
- Providing case management services
- Developing and monitoring support plans
- Assisting with problem-solving and crisis intervention
- Accompanying consumers to appointments
- Acting as an advocate for consumers
- Performing other duties, as needed
- 1+ year of related work experience
- High School Diploma / GED
- Solid assessment and documentation skills
- Patient oriented
- Great interpersonal and communication skills
- Strong attention to detail
- Highly organized
- 3 years of experience with Behavioral Health populations
- Bachelor's Degree
- Experience with Scattering Site settings
- Solid understanding of Supportive Housing dynamics
- Experience with AWARDS
