AAPS/ABPS accepted. Licenses: Active unrestricted NC medical license and DEA w/ NC address & schedules 2,2N,3,3N,4 &5 DEA / CDS: Active DEA with NC address and CDS (schedules 2,2N,3,3N,4,5) Certifications: ACLS, PALS, ATLS preferred; required if not ABEM boarded. Experience: Recent grads accepted Other Qualifictions: CME Attestion form or proof of CME documets last 2 years; Case logs last 2 years; Health Status Statement completed by other provider.
