Participate in quality initiatives aimed at reducing hospitalizations and improving outcomes. What You’ll Bring Education: PharmD or Bachelor of Pharmacy. Experience: Case management, MTM, or managed care preferred. Hospital, ambulatory, and community pharmacists with strong counseling backgrounds are encouraged to apply.
