We believe in dignity, comfort, and continuity of care—wherever our patients call home.Role OverviewWe are seeking a skilled and empathetic Nurse Practitioner (NP) or Physician Assistant (PA) with experience in palliative care to join our mobile medical team. This role is ideal for a clinician who values autonomy, meaningful patient relationships, and interdisciplinary collaboration.ResponsibilitiesProvide comprehensive palliative care to patients with chronic or life-limiting illnessesConduct in-home assessments, develop care plans, and manage symptomsCollaborate with physicians, caregivers, and facility staff to ensure continuity of careEducate patients and families on disease progression, treatment options, and end-of-life planningDocument visits and maintain accurate medical records in compliance with regulatory standardsParticipate in interdisciplinary team meetings and contribute to quality improvement initiativesQualificationsActive NP or PA license in the state of New JerseyCertification in palliative care or relevant experience strongly preferredMinimum of 2 years clinical experience in geriatrics, hospice, or chronic disease managementStrong communication and interpersonal skillsAbility to work independently and travel to patient locationsReliable transportation and valid driver’s licenseWhat We OfferCompetitive hourly rate of $80/hourConsistent 40-hour work weekFlexible scheduling within daytime hoursSupportive team environment with clinical oversightOpportunity to make a profound impact on patients’ quality of life #J-18808-Ljbffr LIFE MEDICAL GROUP PC
