Role Overview We are seeking a skilled and empathetic Nurse Practitioner (NP) or Physician Assistant (PA) with experience in palliative care to join our mobile medical team. This role is ideal for a clinician who values autonomy, meaningful patient relationships, and interdisciplinary collaboration. Responsibilities Provide comprehensive palliative care to patients with chronic or life-limiting illnesses Conduct in-home assessments, develop care plans, and manage symptoms Collaborate with physicians, caregivers, and facility staff to ensure continuity of care Educate patients and families on disease progression, treatment options, and end-of-life planning Document visits and maintain accurate medical records in compliance with regulatory standards Participate in interdisciplinary team meetings and contribute to quality improvement initiatives Qualifications Active NP or PA license in the state of New Jersey Certification in palliative care or relevant experience strongly preferred Minimum of 2 years clinical experience in geriatrics, hospice, or chronic disease management Strong communication and interpersonal skills Ability to work independently and travel to patient locations Reliable transportation and valid driver’s license What We Offer Competitive hourly rate of $80/hour Consistent 40-hour work week Flexible scheduling within daytime hours Supportive team environment with clinical oversight Opportunity to make a profound impact on patients’ quality of life #J-18808-Ljbffr Life Tech
