Hospital-to-Home Transition Coordinator - Albany Region

Capital District Physicians Health Plan, Inc. is seeking a Transition Coordinator to support the Hospital to Home program. In this role, you'll collaborate with medical and behavioral health care managers to ensure hospitalized members leave with comprehensive discharge plans. Your work will involve face-to-face and electronic communication with members, administrative tasks, and outreach related to care opportunities. The ideal candidate will have strong customer service skills, and proficiency in Microsoft Office, and an associate’s degree or equivalent experience.
