Housing Stability Case Manager
Magnificat Houses, Inc.
Magnificat Houses, Inc. is seeking a Case Manager to support chronically homeless and disabled adults, promoting stability and self-sufficiency through individualized service plans, benefits coordination, and community partnership referrals. You will conduct in-person and phone consultations, maintain HMIS data, organize participant files, and collaborate with on-site staff to move participants toward mainstream housing.
