Lead Case Manager, Case Management (Day Shift)ARMStaffing is looking for a Lead Case Manager, Case Management (Day Shift) in Jacksonville, Florida.Location: Jacksonville, FLAssignment Duration: This is a 13-week contract position.Schedule/Shift: Day shift from 8:00 AM to 4:00 PM.Job Description: North Florida Rehabilitation Hospital is a state-of-the-art, 50-bed inpatient rehabilitation hospital that provides specialized treatment for patients recovering from severe injuries or illnesses. As a Lead Case Manager, you will play a critical role in overseeing individualized rehabilitation programs tailored to the needs of patients recovering from strokes, brain injuries, neurological conditions, trauma, spinal cord injuries, amputations, and orthopedic injuries.Responsibilities:Lead and coordinate case management for patients in the rehabilitation program.Collaborate with interdisciplinary team members to create and implement care plans.Monitor patient progress and adjust treatment plans as necessary.Ensure compliance with regulatory requirements and best practices in rehabilitation care.Facilitate communication between patients, families, and healthcare providers.Required Experience: Minimum of 2 years of social work or case management experience in an inpatient setting, preferably in an acute or rehabilitation hospital. Previous leadership or supervisory experience is strongly preferred.Certifications/Licensure: Current Registered Nurse or Social Work licensure; a Master's or Bachelor's in Social Work or licensure as a healthcare professional such as a Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist.
