Job Description – Licensed Practical Nurse (LPN) – Part-Time Position: Licensed Practical Nurse (LPN) Schedule: 16–20 hours per week (Tuesdays and Thursdays) Location: On-site Boothwyn Pennsylvania 19061 Compensation: $26–$30 per hour, based on experience Position Summary We are seeking a reliable and compassionate Licensed Practical Nurse (LPN) to join our team on a part-time basis. This role requires working on-site in our Delaware office and collaborating with clinical staff to support patient care and care coordination activities. Key Responsibilities Provide nursing support and patient care services in accordance with established clinical protocols. Perform patient assessments and document findings accurately. Assist with care coordination and follow-up activities. Maintain accurate and timely documentation in electronic medical records. Communicate effectively with patients, caregivers, and interdisciplinary care teams. Support ongoing patient education and chronic disease management initiatives. Ensure compliance with all applicable healthcare regulations and organizational policies. Required Qualifications Active and unrestricted LPN license in the state of practice. Graduate of an accredited Practical Nursing program. Current CPR/BLS certification. Strong assessment, communication, and documentation skills. Ability to work independently while maintaining effective collaboration with clinical teams. Proficiency with Electronic Medical Records (EMR/EHR) systems preferred. Preferred Qualifications Minimum 1 year of clinical nursing experience. Experience in home health, infusion therapy, chronic care management, or related settings preferred. Work Schedule 16–20 hours per week. Tuesdays and Thursdays. On-site attendance required on scheduled workdays.
