RN: Case Manager

Job Title: RN: Case Manager Address: High Point, NC Responsibilities Perform comprehensive assessment of high-risk members. Collaborate with primary care providers to implement individualized care plans. Address and continuously reassess appropriate levels of care. Coordinate care transitions with healthcare professionals. Keep detailed records of clinical, functional, and fiscal outcomes. Identify opportunities for health promotion and illness prevention. Shift Requirements: Monday thru Friday, 8 AM - 5 PM. On‑Site Work Location. License Requirements NC, Compact License accepted. Education and Credentials Associates or Bachelor's Degree in Nursing or related field. Current, unrestricted state Registered Nurse license. Case Management certification. Benefits Benefits are available to full‑time employees after 90 days of employment. A 401(k) with company match is available after 1 year of service. Equal Opportunity Employer.
