Care Manager Mobile Response Team (MRT)

A Care Manager for our Mobile Crisis Response Team willprovide direct care to individuals served with behavioral health needs, including supporting individuals served with coordination and continuity of evidence-based care for individuals and families in need through a collaborative triage, assessment, planning, and communication process. The position requires traveling within Hillsborough County in a personal vehicle or agency vehicle to facilitate in-home client services in various socioeconomic neighborhoods. Must be able to lift and carry up to 50lbs of dead weight Must have excellent interpersonal skills Minimum of 21 years old or older, possess a valid Florida Driver's license, reliable personal transportation, and insurance. POSITION RESPONSIBILITIES Completion of thorough assessments and coordination of community resources based on patient needs as per program requirements. Complete all clinical and administrative documentation based on established timelines as per program requirements. Communicate effectively and maintain cooperative relationships with internal and external resources. Develop service plans, treatment plans, and/or aftercare plans with clear goals and objectives as per program requirements. Maintain efficient workflow management and organization as per program requirements. Maintain proficient quality of clinical documentation as per program requirements. Other duties as assigned or needed as per program requirements. WORK ENVIRONMENT: Secure facility that provides inpatient treatment programs for those experiencing a mental health emergency needing 24-hour care in a safe, supportive environment.
