Shift Type:  Monday-Friday, 8:30 AM - 5:00 PM

Hours per Week:  40

Candidate Pay Rate:  $53 per hour

Roles & Responsibilities

• Coordinate seamless care transitions for patients across the hospital
• Collaborate with physicians and social workers to create effective discharge plans
• Utilize Midas and Meditech systems to manage case documentation
• Advocate for patients while ensuring appropriate resource utilization
• Support complex discharge planning needs with creativity and compassion

Qualifications

• Active RN license in Florida or compact license
• At least 1 year of case management experience
• Hands-on experience with Midas and Meditech required
• Strong background in discharge planning preferred

Knowledge & Skills

• Proficient with Midas and Meditech electronic systems
• Skilled in complex discharge planning strategies
• Excellent communication and collaboration abilities
• Detail-oriented with strong organizational skills
