- Value-based care model \n
- Medicare/Medicaid Advantage patients \n
- All patients are 65+ \n
- Monday-Friday office only schedule \n
- Phone call shared with all providers \n
- Minimal phone call once every few months \n
- 12-15patients per day \n
- Full supports staff including NP-MA-RN \n
- On-site labs and radiology \n
- \n
- Total compensation $300,000 and above year one \n
- Attainable 20% salary bonus plan \n
- Relocation allowance \n
- Matching 401k retirement plan \n
