Case Manager QIDP - I/DD Services Specialist
Association-Individual Development
The Association for Individual Development (AID) in Elgin, IL is seeking an experienced Case Manager QIDP to coordinate care for individuals with I/DD across Health, Behavioral Health, Employment, and Transportation services. You will manage a caseload of up to 20 clients, document services, lead planning meetings, and ensure compliance with DHS, HCBS, and CARF standards. Flexible hours within a Monday–Friday schedule, with some evenings/weekends as needed to support client needs; the role #J-18808-Ljbffr Association for Individual Development
