Registered Nurse, Care Management

Position Overview RN Float Nurse Case Manager – Case Management Department, Boston Medical Center. Key Responsibilities Utilize a collaborative process to assess, plan, implement, monitor, and evaluate care options and services required to meet individual health needs, using communication and available resources to promote quality, cost‑effective outcomes. Help identify appropriate providers and facilities throughout continuum of services while ensuring efficient use of resources to obtain optimum value for the patient and reimbursement source. Support the overall care management process and department by handling increased volume and providing dedicated assistance. Education and Licensure Graduate of an accredited ASN/BSN Nursing program. Ability to receive BSN within four years of hire date. Licensed to practice as a Registered Nurse in the Commonwealth of Massachusetts. CCM or related case‑management certification preferred, to be attained within 24 months of hire date. Experience 3-5 years diversified clinical experience required. Minimum 2 years of prior experience involving judgment and decision‑making, preferably in a utilization management or case‑management position. Knowledge and Skills Extensive background in current medical/surgical patterns of practice. Proficient in medical terminology and standard medical abbreviations. Understanding of managed care concepts and various health care delivery systems. Ability to assess physical and mental impairment levels and interpret clinical information using ICD‑9/CPT nomenclature. Knowledge of benefits and plan contracts, eligibility issues and requirements. Knowledge of the role and responsibilities of a case‑management professional and the program philosophy within Managed Care.
