The Lexington Health and Rehabilitation Center is committed to providing exceptional care to our residents, and we are seeking a compassionate, motivated, and clinically skilled Physical Therapist (PT) to join our rehabilitation team. As a Physical Therapist at Lexington, you will play a key role in restoring mobility, improving functional independence, and enhancing the overall quality of life for our residents.

About the Role:

We are seeking a licensed Physical Therapist (PT) in the state of Florida (or eligibility for licensure) to join our dynamic rehabilitation team. As a Physical Therapist at Lexington, you will:

• Evaluate residents to determine functional needs and develop individualized treatment plans
• Provide high-quality physical therapy services, including therapeutic exercises, mobility training, neuromuscular re-education, and wound prevention strategies
• Collaborate with an interdisciplinary team, including OT, SLP, nursing, and physician partners
• Document evaluations, progress notes, screenings, and discharge summaries in accordance with regulatory and facility standards
• Participate in care plan meetings and contribute to overall resident-centered care
• Educate residents, families, and caregivers on safe mobility techniques and home exercise programs

Responsibilities:

• Evaluate residents to determine functional needs and develop individualized treatment plans
• Provide high-quality physical therapy services, including therapeutic exercises, mobility training, neuromuscular re-education, and wound prevention strategies
• Collaborate with an interdisciplinary team, including OT, SLP, nursing, and physician partners
• Document evaluations, progress notes, screenings, and discharge summaries in accordance with regulatory and facility standards
• Participate in care plan meetings and contribute to overall resident-centered care
• Educate residents, families, and caregivers on safe mobility techniques and home exercise programs
• Maintain compliance with state and federal guidelines, including Medicare and SNF regulatory requirements
• Assist with developing long-term care programs to ensure residents maintain highest level of function and independence

Qualifications:

• Licensed Physical Therapist (PT) in the state of Florida (or eligibility for licensure)
• Graduate of an accredited Physical Therapy program (DPT or equivalent)
• Previous SNF experience preferred, but new grads are welcome to apply
• Strong communication, organizational, time management, team player, and clinical reasoning skills

About Us:

We are a large 156-bed resident-focused Skilled Nursing Facility dedicated to providing compassionate, evidence-based rehabilitation services. Our team works collaboratively to ensure each resident receives personalized care that maximizes function, safety, and independence. Lexington is currently undergoing an extensive renovation to enhance our residents' environment. This renovation will be completed this year. The rehabilitation department consists of 18 therapists and assistants.
