Temp - PT - Long Term Care (LTC) (Days)

Location: Ada, OK Start Date: 06/15/2026 Hourly Pay Rate: $55.61

Effectively screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care, developing appropriate care plans following all regulatory and clinical practice standards.

Utilize standardized assessments and evidence-based practices to support clinical interventions.

Administer various procedures as part of the rehabilitation plan, including manual techniques; ambulation; therapeutic exercises; modalities; use of supportive and assistive devices; wound care; etc.

Assess patient progress and adjust treatments accordingly, documenting in the clinical record per regulatory and clinical practice requirements.

Evaluate treatment outcomes for effectiveness, modifying services in relation to patient evaluation and referrals based on outcomes.

Provide consultation and counseling to patients, their families, caregivers, and other service providers regarding physical disorders.

Communicate effectively with supervisors and other healthcare team members about patient progress, barriers, and treatment plans.

Prepare comprehensive discharge summaries of services provided, complying with regulatory and clinical standards.

Document all required information in electronic documentation systems and maintain patient records according to company policies.

Accurately record treatment minutes and services provided in accordance with all regulatory and clinical requirements.

Participate in required meetings such as patient care conferences, utilization review meetings, rehabilitation meetings, etc.

Engage in facility and company trainings, in-services, and conferences as required.

Obtain necessary DME or supplies following regulatory and clinical practice standards.

Supervise and support physical therapist assistants, rehabilitation aides, and students.

Adhere to APTA guidelines and State practice acts, codes of conduct, and ethics during professional practice.
