What You'll Do:
As a Physical Therapist in our Long Term Care program, you'll assess, plan, and implement personalized treatment plans for patients with physical functioning disorders and wound care needs. Utilize your expertise to administer therapies including manual techniques, ambulation, therapeutic exercises, modalities, and assistive devices. Monitor patient progress, adjust treatments accordingly, and collaborate with a multidisciplinary team to ensure optimal outcomes. Your role also involves patient education, documentation, discharge planning, and supervising support staff-all while adhering to regulatory and professional standards.


Required Skills:

• Valid state licensure as a Physical Therapist
• Solid knowledge of rehabilitation techniques, wound care, and assessment tools
• Strong clinical judgment and problem-solving skills
• Excellent communication skills to coordinate with patients, families, and team members
• Ability to understand and follow regulatory and clinical practice standards
• Proficiency in electronic documentation systems

Nice to Have Skills:

• Experience in long-term care or skilled nursing facilities
• Knowledge of DME procurement and supplies management
• Prior supervisory experience with PTA or rehab aides
• Familiarity with current APTA guidelines and state practice acts

Preferred Education and Experience:

• Bachelor's or Master's degree in Physical Therapy (DPT preferred)
• Previous experience in long-term care settings is highly desirable
• At least 1 year of clinical experience in rehabilitative therapy

Other Requirements:

• Ability to work daytime hours starting from June 15, 2026
• Willingness to participate in ongoing training and team meetings
• Must meet all regulatory and company-specific clinical documentation and safety standards
• Travel or supply reimbursements available when applicable
