Regular Full-Time ST,CMA,LPN,Q Portland, OR, US 5 days ago Requisition ID: 2025 Location: David Douglas School District | Portland, Oregon Employer: Trillium Family Services Schedule: Full-Time, 34 hours/week Hours: Monday–Friday, School Hours Position Summary Trillium Family Services is seeking a compassionate, organized, and collaborative Outpatient/Prevention Skills Trainer to join our Metro Outpatient Team. This unique position provides behavioral health support within the David Douglas School District while also supporting outpatient services through case management activities. The position is scheduled during school hours, Monday through Friday, and includes: 12 hours/week providing behavioral support in three elementary schools, including direct services in Behavior Support Classrooms. 22 hours/week supporting the Metro Outpatient Team with case management, care coordination, documentation, and client support activities. The ideal candidate is passionate about helping children and families, enjoys working collaboratively with schools and community partners, and is committed to trauma-informed, strengths-based care. Direct Client Services Ensure the safety, security, and confidentiality of clients and families. Implement crisis intervention strategies and crisis plans as directed by supervisors or emergency personnel. Model, coach, and teach behavioral, emotional regulation, communication, and social skills identified in individual treatment plans. Provide individual, family, and group skills training based on treatment goals. Support clients in home, school, and community settings as clinically indicated. Monitor and document client progress toward treatment goals according to agency standards. School-Based Collaboration Serve as a liaison between Trillium, school personnel, families, and community partners. Support the integration of treatment goals within educational settings. Participate in school meetings when appropriate to promote client success. Demonstrate an understanding of educational systems and function as an effective member of school-based teams. Provide behavioral consultation and modeling of effective behavior management techniques for caregivers and school staff. Case Management & Care Coordination Support the Metro Outpatient Team with case management activities. Coordinate services with families, schools, and community agencies. Assist families in accessing community resources and behavioral health services. Maintain community partnerships and referral resources. Ensure timely, accurate, and professional documentation in accordance with agency standards. Professional Responsibilities Maintain client confidentiality in all settings. Participate in weekly individual supervision, group supervision, and outpatient team meetings. Complete required agency trainings. Meet agency productivity and documentation standards. Model professionalism, effective communication, collaboration, and ethical decision-making. Minimum Qualifications Education Bachelor's degree in Psychology, Social Work, Counseling, Behavioral Sciences, or a related field required. Must qualify as a Qualified Mental Health Associate (QMHA) under Oregon guidelines. Experience Minimum of two (2) years of direct care experience working with at-risk children and youth in a behavioral health or treatment setting. Licenses & Requirements Valid driver's license. Driving record acceptable to agency standards. Ability to use a personal vehicle for agency business with current automobile insurance, when applicable. Preferred Qualifications Experience providing behavioral health services in school settings. Experience collaborating with multidisciplinary teams. Knowledge of trauma-informed care and Positive Behavioral Interventions and Supports (PBIS). Strong organizational, communication, and documentation skills.
