Youth & Adult Service Director Position Summary The Youth & Adult Service Director is responsible for coordinating, monitoring, and supporting services for individuals receiving waiver and community-based supports. This position ensures compliance with BDDS, CARF, Medicaid Waiver, and Full Potential policies while promoting person-centered services that enhance the health, safety, independence, and quality of life of each individual served. The Service Director conducts quarterly and annual home visits, monitors Individual Support Plan (ISP) outcomes, collaborates with families and providers, documents services accurately, and ensures all required reporting and follow-up activities are completed within established timelines. Essential Duties and Responsibilities Service Coordination & Home Visits • Conduct scheduled quarterly and annual home visits with individuals and families, including scheduling future meetings, coordination, and prior meeting check-in with the families and individuals. • Complete any necessary IRs and GERs for individuals • Conduct intake of new individuals, including but not limited to: Therap set up, creating profiles, meeting with families, and discussing services • Communication with the Billing Director regarding hours and service utilization • Review Individual Support Plans (ISPs) and monitor progress toward established outcomes. • Assess the individual's overall health, safety, emotional well-being, and quality of supports. • Observe the home environment for safety, accessibility, and environmental needs. • Maintain confidentiality, professionalism, and person-centered practices during every interaction. • Build positive relationships with individuals, families, guardians, providers, and community partners. Health & Medical Monitoring • Review medications with individuals, guardians, and caregivers. • Monitor medication changes and ensure documentation is supplied to the on-staff nurse for updates and tracking within the Therap system. • Discuss physicians, specialists, therapy, hospital, and emergency room visits. • Identify health concerns that may impact services or supports. • Communicate significant medical updates to appropriate team members as required. Behavioral & Emotional Supports • Review Behavior Support Plans when applicable. • Monitor behavioral progress and identify new concerns, triggers, or support needs. • Collaborate with caregivers and behavioral professionals to ensure effective interventions. • Recommend additional behavioral supports and resources when appropriate. Family & Caregiver Support • Listen to caregiver concerns with compassion and professionalism. • Provide education regarding disability services and available community resources. • Identify caregiver training needs and coordinate appropriate supports. • Discuss respite services, caregiver wellness, and additional family support resources. • Maintain professional boundaries while offering encouragement and advocacy. Individual Planning & Service Monitoring • Monitor progress toward ISP goals and outcomes. • Identify changing needs requiring service modifications. • Recommend additional services, therapies, equipment, or environmental modifications. • Coordinate follow-up with providers to ensure continuity of care. Documentation & Compliance • Complete accurate and timely documentation immediately following each home visit. • Prepare comprehensive quarterly reports using approved agency formats. • Document health updates, behavioral observations, family concerns, progress toward goals, resources provided and required follow-up actions. • Upload required documentation into Therap, the State BDDS reporting system, and agency files within required timelines. • Maintain complete and compliant records that meet BDDS, CARF, Medicaid Waiver, and agency standards. Follow-Up & Case Management • Ensure medication, medical, therapy, and service changes are communicated and documented. • Coordinate environmental modifications and adaptive equipment needs. • Provide behavioral, educational, and community resource information to families. • Document all follow-up contacts, communications, and interventions. • Monitor completion of recommended actions until resolved. Quality Assurance & Professional Standards • Demonstrate ethical decision-making and always maintain confidentiality. • Uphold agency values: dignity, respect, compassion, and person-centered care. • Observe for unmet or unspoken needs and advocate for appropriate supports. • Maintain compliance with all federal, state, CARF, BDDS, Medicaid Waiver, and Full Potential policies. • Participate in required training, meetings, and quality improvement activities. • Offer faith-based support, including prayer, only when requested or welcomed by the individual
Requirements:Qualifications • Knowledge of Medicaid Waiver services, BDDS regulations, and person-centered planning preferred. • Experience working with individuals with intellectual and developmental disabilities preferred. • Strong written and verbal communication skills. • Excellent organizational and time-management abilities. • Ability to maintain confidential documentation and accurate records. • Valid driver's license, reliable transportation, and ability to travel for home visits. • Ability to pass all required background screenings.
