Physical Therapist Location: Adrian, MI Agency: Care Career Pay: Competitive weekly pay (inquire for details) Shift Information: Days - 5 days x 8 hours Start Date: ASAP About the Position Key Responsibilities Provide physical therapy services to patients in their homes in accordance with physician orders and care plans Perform patient evaluations and start-of-care visits Develop and implement individualized therapy treatment plans Educate patients and caregivers on exercises, mobility techniques, and safety strategies Document patient progress and treatment outcomes in the EMR system Collaborate with nurses, physicians, and interdisciplinary care teams Complete required clinical documentation including OASIS assessments when applicable Clinical Support & Workflow PTs focus primarily on evaluations and start-of-care visits Physical Therapist Assistants (PTAs) assist with follow-up visits Dedicated scheduling team manages patient visit scheduling EMR system: Homecare Homebase (HCHB) Requirements Qualifications Licensed Physical Therapist (PT) Current Michigan PT license required Home health experience preferred OASIS documentation experience preferred Experience with Homecare Homebase (HCHB) preferred Strong clinical assessment and patient education skills Ability to work independently while managing a patient caseload Benefits Compensation & Incentives Base Salary: $100,000 – $120,000 (Based upon Experience) $10,000 Sign-On Bonus Productivity Bonus Structure Expected productivity: 27 points per week Additional compensation of $65–$75 per point for productivity above target Annual bonus opportunities Company vehicle available for therapists driving 1,400+ miles per month #J-18808-Ljbffr
