Job Title: Center Manager FLSA Status: Exempt Job Summary: Under the general supervision of the Chief Operations Officer, the Center Manager will have authority and responsibility for the management of Interdisciplinary Team activities and staff. The Team Lead will fulfill the regulatory role of the Center Manager. The Team Lead/Center Manager will ensure that decisions about medical, social, and supportive services are driven by the IDT decision‑making process and not influenced by the fiscal manager. All employees are expected to be knowledgeable and compliant with Pace of Southern Piedmont values of compassion, dignity, excellence, integrity and teamwork. Specifications Education: Associates Degree in Health Care Related Field. Experience: 5 years of experience in health care management or nursing with at least 2 years supervisory experience. Minimum of 1 year experience working with the frail elderly population. Number and Type of Employees Supervised: Members of the Social Work, Home Care, Therapy, Dietary, Transportation and Administrative Support. Licensure, Registry or Certification Required: Valid NC Driver’s License and vehicle. Professional licensing as required by position. Special Training: Must possess the skills necessary to provide care for geriatric patients, must thrive in a team environment, must possess good organizational and supervisory skills, and ability to effectively handle difficult and unusual interpersonal situations. Only act within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by Pace of Southern Piedmont and approved by CMS before working independently. Immunizations: Be medically cleared for communicable diseases and have all immunizations up‑to‑date before engaging in direct participant contact. Ages Of Patients Rendered Care: Neonate/Infant, Early Childhood, Adolescent, Adult, Geriatric, All Age Groups. Key Responsibilities Embraces the goals to enhance the function, mobility, independence, quality of life, and quality of end of life of participants and holds direct reports accountable to support the organization’s goals. Functions as a member of leadership within the Pace of Southern Piedmont and promotes the mission and vision of leadership in a positive and productive manner. Directs and oversees the IDT team members. Hires, trains, evaluates and disciplines IDT Team Members in coordination with the Chief Operations Officer. Coordinates staff development and training, and assists with other human resource issues in conjunction with GFHS Human Resources Department. Assists with the professional development and supervision of all IDT disciplines including Home Care Coordinators, therapy service providers, Restorative Aides, Social Workers, Dietitian, transportation representatives, and administrative support staff, meeting regularly with direct reports to review goals, trends and performance measures to facilitate change as needed. Provides input, in conjunction with the Medical Director, in PCPs’ annual evaluations as it relates to IDT and team performance as well as with Clinic Manager for Clinic RNs’ annual evaluations and the Therapy Services Manager for the therapy staff’s annual evaluations as it relates to IDT and team performance. Ensures that team members understand the importance of their and others’ roles on the team and remain open to the cooperative dynamic of the team. Emphasizes teamwork and collaboration in all clinic and Interdisciplinary interactions. Coordinates orientation and in‑service programs for staff to meet regulatory requirements and support performance improvement. Ensures the provision of quality service within expected standards and financial targets. Takes responsibility for the development, implementation, and execution of participant care. Facilitates activities related to new participants, in coordination with the IDT Team, including new participant orientation. Ensures that participants are oriented to the program and have opportunities to express their opinions about the care and services they receive. Coordinates all IDT Team Meetings including care planning and implementation; preparation for interdisciplinary conferences, daily meetings, and family conferences; and all related follow‑up. This includes the performance of routine care plan audits and all related follow‑up with disciplines’ interventions and evaluations in participants’ care plans. Participates in the continued development of the team to improve the team decision‑making process, providing support to the team to troubleshoot, advise, teach, coordinate and deliver care and service to participants. Participates in the Pace of Southern Piedmont Quality Program by collaborating with the Quality Coordinator in support of the Service Delivery and Grievance resolutions and education of said SDAGs to direct reports. Provides participant monthly data to Quality Coordinator for Quality reports. Serves as liaison between individual program disciplines and community service providers. May fill two separate roles on the interdisciplinary team where the individual meets applicable state licensure requirements and is qualified to fill the two roles and is able to provide appropriate care to meet the needs of participants.
