Nurse Practitioner, Center of Excellence for Alzheimer's Disease

Job Overview Department/Unit: Neurology Unit/Location: Albany Med Center of Excellence for Alzheimer’s Disease Shift: Day (United States) Responsibilities Assess, plan, implement, and evaluate care for individual patients. Assess patient/family needs and provide education. Promote wellness and recommend community resources to meet patient and family needs. Promote/provide staff education. Incorporate evidence-based research into clinical practice. Be knowledgeable of reimbursement procedures and documentation requirements. Provide case management for patients and their families with support from Center social workers. Serve as a resource person for the nursing staff. Mentor new APPs as appropriate. Collaborate with other services (radiology, apheresis, laboratory) to coordinate care for dementia patients undergoing monoclonal antibody infusion treatments. Qualifications Master's Degree from an accredited Nurse Practitioner program – required. Quickly master all aspects of a computerized medical recordkeeping and billing system. Must project a professional image and earn the confidence of others. Excellent interpersonal, verbal, and written skills. HIPAA compliant at all times. NP – Nurse Practitioner – Licensed Upon Hire – required. ATLS – Advanced Trauma Life Support Upon Hire – required. BCLS – Basic Life Support Upon Hire – required.
