Provides guidance to clients with a knowledge of social and vocational case management and counseling methods to develop individualized service plans and perform assessments to determine personal, social, health and economic needs. Assists clients by independently identifying and referring them to available federal, state, local, and private resources to assist with their needs. Educates clients on clinical and other relevant procedures and coordinates client care and community resources. Duties:
Service Delivery
• Conducts client intakes, assessments, and needs analysis.
• Develops and implements care service plans; provides mentoring and links to community resources.
• Educates and supports clients, encouraging participation in meetings and programs.
• Promotes health and safety through program compliance
• Completes documentation accurately and timely, following policies and procedures.
• Reports crisis situations and adheres to critical incident response and reporting protocols Outreach
• Supports outreach, recruitment, and community engagement efforts.
• Builds partnerships and links clients to health education and community resources Professional Development
• Assesses personal training needs annually and participates in relevant educational opportunities.
• Provides education and shadowing experiences for public health students. May be required to perform other duties as assigned. May be required to assist the agency or state government generally in the event of an emergency declaration by the Governor. May be required by VDH to adjust work hours, location, and job duties and participate in training to meet public health business needs, including emergencies. If this position is designated as a mobile worker, this includes both working in the field and teleworking at an alternate location when not in the field or during an emergency closing. You may be required to periodically come into your assigned office to attend meetings or training. • Experience providing support resources and information to customers.
• Understanding of infant and child development and family services.
• Experience in individual counseling, relationship building, and group leadership.
• Strong verbal and written communication skills.
• Skill in interviewing clients to gather, interpret, and use data effectively.
• Proficiency in documentation, data entry, and record-keeping.
• Proficient in using computer systems and web-based case management documentation tools.
