Job Details Primary City/State: Mesa, Arizona Department Name: Home Care Team-East Work Shift: Day Job Category: Clinical Care Position Overview As a Home Health Speech Therapist, you will join the Banner Home Health Clinical Team providing exceptional, expert care in the community. You will have an autonomous practice focusing on functional rehabilitation within the patient’s actual home environment, reflecting the patient’s specific needs. You will create relationships with patients, build your own schedule, and focus on enabling patients to return to daily life by addressing physical and emotional needs during illness and recovery. This position requires a current license. Schedule Training Core Monday‑Friday 8:00am‑5:00pm First 30‑90 days Training BSS/PRN Commit to 1‑2 days per week Monday‑Friday 30‑60 days After Training Option 1: Full Time 40‑32 Hours per week Monday- Friday “Work your way” 4 10hr shifts or 5 8hr shifts Saturday & Sunday + other days during the week “Work your way” 4 10hr shifts or 5 8hr shifts Commitment to working every other weekend with working weekdays on the off weeks, or working every weekend with flexibility of working one week day. Option 2: Part Time 20‑28 Hours per week Monday-Friday: 5, 8, 10 hour days Saturday- Sunday +1 other day 5, 8, 10 hour days Option 3: BSS/PRN PRN Position Call Outs/ PTO/ Leave Sunday- Saturdays 8:00am-5:00pm Minimum of 2 shifts per month Locations East Valley - POD 1 Primary cities: East Mesa, Gold Canyon, Apache Junction, Superior East Valley - POD 2 Primary cities: Mesa, Gilbert, San Tan Valley, Queen Creek, Florence East Valley - POD 3 Primary City: NE Mesa East Valley - POD 6- Primary cities: Casa Grande, Arizona City, Eloy, Coolidge and Maricopa East Valley - POD 8- Primary cities: Phoenix, Mesa, Tempe, Chandler East Valley - POD 9- Primary cities: Phoenix, Scottsdale, Fountain Hills, Cave Creek, and Carefree Position Summary This position provides screening, evaluation, and rehabilitation services for patients with speech and language disorders related to speech, language, cognitive-communication, voice, swallowing, and fluency. The role plans and carries out specialized treatment programs, documents treatment, and maintains data regarding patient care. The position demonstrates clinical competence and knowledge necessary to provide treatment appropriate to the age of the patients served. Core Functions Evaluates patients’ functional abilities and limitations to determine appropriate intervention. Develops a plan of care and treatment goals, as ordered by physician or other referral source. Implements individualized treatment plans that consider patients’ physical, social, spiritual, cultural, educational, and age-specific needs. Ensures that treatment plans are supported by standard practices and/or current literature. Re-evaluates treatment plans, as needed, to ensure maximum patient benefit. Educates patients, family, and other care providers to promote patient participation and attainment of therapeutic program goals. Provides individualized home programs with clear instructions. Documents patient treatment according to professional and regulatory standards. Participates in the provision of training to other staff and students. Follows plan of care to accomplish functions within established procedures and scope of practice. Makes independent judgments based on specialized knowledge. Confers with supervisor on any unusual situations. Internal customers: All levels of nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External Customers: Physician offices, patient families, third party payers, and medical supply companies. Minimum Qualifications Masters degree in Speech Language Pathology from an accredited university program. Current licensure in the state of practice. Certificate of Clinical Competence or documented acceptance in Clinical Fellowship Year. BLS certification required. Requires the ability to work collaboratively with physicians and other healthcare providers. Must have strong interpersonal communications and human relation skills. Must have the ability to become proficient with computer software applications commonly used in an automated patient care environment. Employees must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.
