Schedule:

• Flexible shifts available Monday through Sunday
• Self-directed schedulingchoose your days and hours
• Minimum hourly commitment weekly or bi-weekly

Key Responsibilities:

• Conduct thorough, approximately 45-minute home health risk assessments for Medicare Advantage members
• Evaluate patient histories, existing medical conditions, and medication use
• Deliver proactive health and medication education tailored to each individual
• Pinpoint care gaps and recommend appropriate actions
• No requirements for creating treatment plans or prescribing medications

Qualifications:

• Nurse Practitioner License in New Mexico
• Board Certified by AANP or ANCC
• Current BLS/CPR certification
• MSN from an accredited nursing program
• At least 6 months of recent NP practice (preferred)

This role offers compensation of $80-$90 per completed evaluation, including a mileage stipend and a paid administrative time component.
