Job Summary As a member of a multidisciplinary health‑care team, the clinical pharmacist provides clinical, consultative, research, and/or administrative pharmaceutical services for NICU, Obstetrics, Acute Care, OR, and SSU. Clinical Pharmacists I select, compound, dispense drugs and medicines, consult with nursing staff and physicians regarding medication orders, disseminate drug information to hospital staff and community groups, and may provide recommendations to pharmacy leadership regarding new drug requests and non‑formulary applications. Essential Functions Employees must abide by all Joint Commission requirements, including cultural diversity, patient rights, safety, teamwork, ongoing education, and adherence to safety and quality programs. They must also follow hospital policies, Service Standards, and the Code of Conduct. Identifies differences between drugs as they relate to therapeutic efficacy. Applies knowledge of the physical and chemical properties and characteristics of drugs, including dosing calculations, weighing and measuring, and compounding. Evaluates solubility, stability, usage, and limitations of drug administration, and identifies incompatibilities to prevent adverse reactions. Has legal responsibility for proper prescription interpretation and dispensing, and alerts physicians to synergistic, antagonistic, or cumulative effects, questionable overdoses, or incompatibilities. Specifies proper storage and protection of therapeutic agents per US Pharmacopoeia and package inserts. Stays abreast of new developments in pharmacology and emerging patterns, systems, and techniques. Participates in training, preceptorship, and education of pharmacists, residents, interns, technicians, and pharmacy students. Maintains records, documents patient data, prepares written reports, and provides oral reports concerning patient care. Monitors patient medication therapy to ensure safe and effective therapy. Provides drug information to physicians, nurses, and other health‑care professionals related to medication therapy. Verifies orders for non‑advanced areas. Mays serve and participate in departmental and interdisciplinary committees. Supervises pharmacy technicians. Prepares sterile and non‑sterile products, checks and dispenses them. Manages and controls controlled substances. Participates in emergency events requiring a licensed pharmacist (Rapid Response, Code Blue Resuscitation). Higher‑Level Functions (under supervision) Performs verifications for advanced practice areas, as defined by Pharmacy leadership. Assists with discharge medication coordination, including prior authorizations and discharge medication education to patients. May be present for planned bedside procedures to provide stat medications as needed. Provides direct consult with providers regarding patients in covered areas. Performs drug monitoring of patients and may make appropriate changes to orders per policy; may participate in department initiatives and projects as assigned. Provides in‑service education to pharmacy, nursing, medical residents, fellows, and physicians on medications. Qualifications Minimum Qualifications Any combination of education and experience that provides the required knowledge, skills, and abilities, and possession of any required licenses or certifications. Education: Doctor of Pharmacy degree from an accredited School of Pharmacy. Experience: Two (2) years of related hospital experience. License/Certification: Registered Pharmacist by California Board of Pharmacy RQI Healthcare Provider Basic Life Support or Basic Life Support by American Heart Association (AHA) RQI Healthcare Provider Pediatric Advanced Life Support or Pediatric Advanced Life Support by American Heart Association (AHA) within 90 days of hire Knowledge Knowledge of prescription and non‑prescription drugs, their indications, efficacy, dosage, mechanisms of action, disposition, contraindications, and side effects. Knowledge of medical terminology, pharmacology, and pharmacokinetics. Knowledge of proper handling and storage of pharmaceutical supplies. Knowledge of how patient factors (age, sex, disease state, concurrent therapy, foods, diagnostics) affect drug activity and disposition. Knowledge of physiology, anatomy, pathology, chemistry, and mathematics as they relate to pharmacy practice. Knowledge of local, state, and federal regulatory requirements. Ability to apply judgment and make informed decisions. Ability to plan, organize, prioritize, work independently, and meet deadlines. Ability to foster effective working relationships and build consensus with other departments and external vendors. Ability to communicate effectively and exercise tact with hospital staff, patients, and other professional contacts. Maintain familiarity with departmental information and technical systems. Ability to work with devices, information systems, and automated equipment used in pharmacy. Additional Information Compensation is based on the level and requirements of the role. Salary may also be determined by education, experience, knowledge, skills, location, and internal equity. Typically, new team members join at the minimum to mid salary range.
