Bilingual Care Navigator & Referral Coordinator

Harmony Healthcare Long Island in Hempstead, NY, is seeking a Patient Navigator for a full-time role. You will coordinate referrals for Specialty Services and Diagnostic Imaging, obtain pre-authorizations, track referral status, and facilitate care coordination with the clinical team. The position requires strong communication, organization, and EHR proficiency.
