Care Coordinator

Care Coordinator The Care Coordinator is responsible for the oversight of the residents and staff in the area they are assigned to. This position will manage all clinical aspects of their neighborhood while promoting harmonious relationships and favorable customer relations among nursing staff and other departments. Hours for this role will typically be Monday-Friday 8:00am-4:30pm. Hours may vary depending on the needs of the department. Job Duties Provide 24-hour management of assigned neighborhood. Identify neighborhood concerns and initiate resolution of concerns. Be knowledgeable and keep self and staff updated concerning Federal nursing home rules and regulations, Standards of Nursing Practices in LTC, Residents Bill of Rights, Nursing Department policy and procedures, Shorehaven Mission Statement and Employee Policy Handbook. Oversee staff nurses in assessing residents and identifying problems (using Nursing Diagnosis); formulate resident goals and nursing intervention (Nursing Process). Identify emotional and social needs of residents and coordinate plans of care with appropriate department such as Social Services, Chaplaincy or Activity. Create and maintain a calm environment and atmosphere of warmth and personal interest throughout the neighborhood. Assist in training staff. Guide and orient staff with respect to quality of care and quality of life. Supervise, counsel and evaluate assigned staff. Participate in the development of policies and procedures to improve the quality of the delivery of care system. Communicate effectively with Director of Nursing, staff and other departments and families (i.e.: household meetings, care planning conferences, shift reports, family conferences). Other duties as assigned. Qualifications Registered Nurse currently licensed by the State of Wisconsin. Requires knowledge attained in an accredited Registered Nursing program. Twelve (12) months of related work experience. Evidence of basic leadership skills and supervision. Ability to plan, organize, teach and evaluate the performance of others.
