As a Physical Therapist (PT) , you will provide exceptional physical therapy services to our residents in a fast-paced, dynamic environment. Your expertise will help our residents recover from injuries, manage chronic conditions, and maintain their independence. Physical Therapist (PT) will work collaboratively with our interdisciplinary team to develop and implement individualized treatment plans that address the unique needs of each resident.

Responsibilities:

• Conduct physical therapy evaluations, assessments, and treatments to residents with various diagnoses and conditions

• Develop and implement individualized treatment plans that address the physical, functional, and cognitive needs of each resident

• Provide education and training to residents, families, and caregivers on proper exercise techniques, disease management, and prevention strategies

• Collaborate with the healthcare team to ensure seamless care transitions and effective communication

• Maintain accurate and timely documentation of resident progress, treatment plans, and billing information
