Nurse Practitioner (Full-Time)

We are seeking a dedicated Full-Time Nurse Practitioner to provide primary care to elderly participants in a community-based program. Key Responsibilities Include Performing physical assessments and developing individualized care plans. Evaluating participant complaints and administering appropriate treatments. Conducting semi-annual reassessments and in-person evaluations for new enrollees. Collaborating with an Interdisciplinary Team to ensure comprehensive, coordinated care. Providing education and guidance to participants and caregivers. Overseeing residents in skilled nursing facilities and coordinating with primary care providers. Participating in discharge planning and managing emergent and acute needs. Qualifications Include Licensed Registered Nurse (RN) with current state licensure. Graduate of an accredited nursing program. Certified as a Nurse Practitioner by a recognized certifying organization. At least one year of experience working with the elderly population. Training or related experience in the role. Preferred Qualifications Geriatric Nurse Practitioner certification with prescriptive authority. Two or more years of geriatric NP experience. This Role Requires Providing care to elderly residents, including evaluation and treatment. Coordination with healthcare team members and discharge planning. Intermittent local travel to participants' homes.
