Nurse Practitioner- Palliative Care

Summary The John D. Dingell VA Medical Center is recruiting for a Nurse Practitioner - Palliative Care. The NP provides a full range of services in outpatient/inpatient settings emphasizing quality, timeliness, and the advancement of the mission. The NP offers advanced clinical practice, consultation, and management, practicing to the full extent of education, training, and certification. Duties Has prescriptive authority for a panel of patients or program as indicated by position or assigned by supervisor. Independently manages or collaborates with attending physicians a panel of patients with acute and chronic health conditions emphasizing health promotion and disease prevention using evidence-based practice. Develops and implements individualized plans to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with interdisciplinary team. Initiates referrals and consults to specialty services. Provides End-Of-Life care and symptom management to all patients in inpatient and outpatient settings. Assesses and adjusts plan of care as needed to achieve optimal patient outcomes. Committed to delivering compassionate, timely, veteran-centered care through multidisciplinary palliative care teams. Teams support Veterans in VA hospitals, community living centers (CLCs), and remotely via telehealth services. Provides medical services to veterans on an outpatient/inpatient basis or as assigned by supervisor at John D. Dingell VAMC under GEC service.
