- Conduct evaluations for patients in both inpatient and outpatient environments, developing personalized treatment strategies.
- Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function.
- Collaborate effectively with fellow healthcare professionals - including nurses, physicians, and case managers - to ensure optimal patient outcomes.
- Guide patients and their families through therapeutic objectives, discharge plans, and home exercise routines.
- Maintain detailed documentation of evaluations, treatments, and progress in compliance with facility requirements and regulations.
- Engage in interprofessional rounds and team conferences to facilitate coordinated care.
- Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution.
- Current state licensure as a Physical Therapist .
- CPR certification is preferred.
- Specialty certifications (e.g., neurological, geriatric) are advantageous but not mandatory.
- Previous clinical experience is highly preferred.
- Expertise in treating patients with intricate medical and mobility challenges.
- Strong communication and teamwork capabilities.
- Familiarity with digital charting and clinical documentation systems.
