Concerto is seeking a PRN Social Worker to join its team! This role provides psychosocial evaluations, counseling, and quality-of-life support to dialysis patients in Skilled Nursing or rehab facilities. As part of the interdisciplinary team, the PRN Social Worker addresses psychosocial needs, ensures timely care planning per CMS guidelines, and maintains detailed documentation to meet QAPI standards. The position reports to the market Team Lead and indirectly to the Director of Social Services. What Will You Do? Patient Assessment & Care Planning Conduct psychosocial assessments (initial, 90-day, annual, and as‑needed). Administer quality‑of‑life and depression screenings (KDQOL, PHQ‑2/9). Develop individualized care plans in collaboration with the interdisciplinary team. Therapeutic Support & Education Provide counseling and education on dialysis, transplant options, and advance directives. Deliver interventions to support treatment compliance and quality of life. Develop patient education materials. Interdisciplinary Collaboration Lead monthly care plan meetings with patients and clinical staff. Coordinate with facility teams, EQRS specialists, and regional managers. Support new clinic openings and admissions. Documentation & Compliance Maintain accurate EMR documentation of assessments, interventions, and follow‑ups. Track and report productivity and outcomes. Ensure compliance with CMS guidelines and Concerto policies. Advocacy & Resource Coordination Address patient and family concerns. Assist with outpatient dialysis placement and discharge planning. Connect patients with community resources and support grievance reporting. Master’s degree in social work. Healthcare or dialysis related background preferred. Must hold a valid state license; clinical not required. Unencumbered driver’s license. Intermediate computer skills and proficiency with MS Word, Excel, Outlook as well as becoming proficient with Concerto specific applications within 60 days of employment. Physical Requirements & Work Environment The role requires the ability to lift at least 20 pounds and maintain strong mental, visual, and auditory focus when interacting with patients, staff, and customers. The work environment is typical of a healthcare facility, featuring controlled temperatures and moderate noise levels, with potential exposure to infectious or contagious materials. Additionally, the position involves travel between different facilities, necessitating flexibility and mobility.
