About the Role

Help ensure patients receive the coordinated, compassionate care they deserve. As a Physician Services Clinical Coordinator, you'll serve as the central point of connection between providers, patients, caregivers, and healthcare partners, helping navigate complex healthcare needs and ensuring care plans are carried out smoothly and efficiently. Through thoughtful coordination, communication, and follow-through, you'll help create a seamless care experience for elderly and medically complex patients. This role is ideal for someone who thrives on organization, problem-solving, and building meaningful relationships while finding purpose in supporting patients and families. You'll coordinate services, manage provider orders, facilitate communication between care teams, support patient follow-up, and connect patients with the resources needed to support their health and well-being. Your attention to detail, proactive approach, and commitment to exceptional service will directly contribute to continuity of care, strong provider partnerships, and the delivery of high-quality, patient-centered healthcare. Location Home Office: Hours Monday to Friday, 8 a.m. to 5 p.m. Required Qualifications High school diploma or GED CENA certification, MA certification, or equivalent work experience Minimum 1 year of experience in medical field or medical office setting Strong grasp of medical terminology Proficiency in Microsoft Office Suite (Outlook, Word, Excel, PowerPoint) Reliable transportation with valid automobile insurance Preferred Qualifications Associate's degree Experience with geriatric population and/or caregivers Previous experience in care coordination or medical office management Essential Skills Excellent verbal and written communication abilities Exceptional organizational and time management skills Strong problem-solving skills with creative thinking Ability to work well under stress and in urgent situations Professional communication skills across all organizational levels Understanding of communication barriers (hearing loss, cognitive impairment) Ability to triage and prioritize effectively Customer service orientation with empathy for elderly patients Pay Range $18—$21 USD How We Care for You Mileage Reimbursement Minimum of 3 Weeks Paid Time Off for Full Time Part Time/Per Diem also accrue Paid Time Off Medical, Dental, and Vision Insurance HSA and FSA options including Dependent Care Company paid Short Term Disability Company paid Life Insurance 401(k) with Employer Match Pet Insurance ID and Fraud Protection And more...
