**Exciting Opportunity for Physical Therapists in Home Health!** **Job Description:** - Are you a passionate Physical Therapist looking for a fulfilling role in a dynamic home health environment? - Join us in providing exceptional care to patients in the comfort of their own homes, making a real difference in their lives! - Locations available: Missoula, Hamilton, Stevensville, Ronan, and Polson. **Position Details:** - **Role:** Physical Therapist (PT Home Health) - **Status:** Full-Time | 40 hours/week with possible call - **Schedule:** Monday to Friday, 8am-5pm, with rotating on-call, weekend, and holiday coverage. **Key Responsibilities:** - Evaluate patients and establish personalized plans of care to meet their needs in the home setting. - Provide direct patient care according to established plans and physician orders. - Re-evaluate patient status and modify treatment plans as necessary, adjusting frequency and duration of care. - Communicate effectively with physicians and interdisciplinary team members. - Document all skilled services provided to patients accurately. - Coordinate daily with scheduler to ensure consistency of care. - Supervise Physical Therapist Assistants when applicable. - Carry out additional duties as assigned by your supervisor. **Minimum Qualifications:** - Bachelor’s degree or higher from an accredited Physical Therapy program. - Current license to practice in the State of Montana. - Valid driver’s license and appropriate auto insurance. - Preferred: Two years of previous Physical Therapy experience. **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
