The therapist will work in coordination with Silver Lining's outpatient counseling and psychiatric nurses to provide psychotherapy services to older adults with behavioral health issues.The therapist will work in collaboration with the care liaison to coordinate client care, assist with insurance pre-authorization, and caseload management.You will develop alliances with patients, their families, and caregivers.For geriatric clients, you guide patients and families through the challenges of the late life mental illnesses and the journey of dementiaYou will conduct behavioral health diagnostics and crisis evaluations.You will utilize standardized cognitive, behavioral, and mental health assessment tools.Provide ongoing psychotherapy in individual and group settings for patients and caregiversConduct in-home, tele-presentation/virtual, and telephone support services for patients and their families, caregivers.Provide case management to patients and caregivers.RequirementsMinimum Required Education:At least one year providing direct client care.Experience of working with older adultsIndependent LCSW or LCPC in the state of IllinoisSelf-motivated and independentFamiliarity with community mental health resourcesBilingual a plusLicenses/Certifications:Must possess an active license to practice counseling or social work in the State of Illinois: LCPC, LCSWOther Skills Required:Experience in delivering high-quality, client-centered careAble to document and navigate EHR independentlyAbility to maintain strict client confidentialityAbility to effectively communicate and interact with individuals involved in all aspects of programming –children, parents, school faculty and staff, grantors, funders, partner agencies, members of the community and Silver Lining team membersProficient computer skills, including working knowledge of Microsoft Office SuiteAbility to work effectively on multidisciplinary teamLanguage Skills: Ability to read, analyze and interpret general and professional documents. Ability to effectively present information in one on one and small group situationsMust possess the ability to make independent decisions when circumstances warrant such actionAbility to foster teamwork; train and direct the work of team members and colleaguesExcellent organizational skills, including the ability to work productively and make independentWorking Conditions / Environment:1099 or W2 options availableFlexibility in setting work scheduleEssential Duties and Responsibilities:Responsible for providing counseling and psychotherapeutic services that are consistent with licensure and training.to clients in a variety of clinical settings. Assess, diagnose, and treat adults, adolescents, and children experiencing symptoms of behavioral health disorders.Collaborate with counselors, social workers, or other treatment team members to coordinate client care.Responsible for timely and accurate completion of medical records, including assessment, treatment plan,progress notes, outcome measures, and billing entries.Counsel clients during appointments, as necessary.Design/participate in the development of individualized treatment plans, incorporating best and leadingpractices into treatment interventions.Collaborate with guardians, relatives, and significant others on client's conditions and treatment whenclinically appropriate.Prepare and submit assessments, progress notes, reports, and summaries to entities/agencies (funders,insurance companies, etc.) as required.Return calls and emails promptly (within 24 hours of receipt or sooner if clinically necessary)Responsible for making independent decisions based on protocols developed utilizing national guidelines for treatment interventions.Responsible for maintaining customer relations with all agency staff and clients.Records all services performed for accurate billing for services provided in compliance with all local, stateand federal laws, as well as payor rules and regulations. Submits billing documents in a timely fashion.Looks for ways to improve and promote quality.
