Work Environment

This position is based in our home care setting, meaning you'll work directly in patients' homes, allowing for personalized therapeutic interventions in a comfortable environment. Our team operates Monday through Friday from 8:00 AM to 5:00 PM, ensuring a consistent schedule that supports work-life balance. Your role will also include using state-of-the-art tools and virtual platforms to engage with patients, enhancing their recovery experience.

Key Responsibilities

• Conduct initial examinations and evaluations to assess patients' physical capabilities and limitations, formulating tailored treatment plans based on these assessments.

• Implement treatment protocols that utilize the latest techniques and evidence-based practices, focusing on individual patient needs.

• Maintain thorough documentation of patient progress, including evaluations, treatment plans, and discharge summaries, ensuring compliance with local and federal regulations.

• Collaborate with a multidisciplinary team to coordinate care, referring patients for additional medical consultation as necessary.

• Engage patients in their rehabilitation process, providing education on exercises, safety, and health maintenance.

• Manage discharge planning to facilitate smooth transitions for patients returning to everyday activities, while providing follow-up care when needed.

• Participate in ongoing professional development opportunities, team meetings, and in-service training to continually enhance clinical practices.

Patient Population / Client Focus

You will be working with a diverse patient population that includes individuals recovering from surgery, managing chronic conditions, or facing mobility impairments due to injury or illness. Our approach emphasizes respect for each patient's unique circumstances and goals, enabling you to apply your clinical skills in a meaningful way that leads to improved health outcomes.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited PT program.

• Valid state license to practice as a physical therapist in Florida.

• Minimum of 2 years of related experience in physical therapy.

• Current certification in CPR and other relevant certifications.

• Strong interpersonal communication skills that facilitate productive relationships with patients and healthcare team members.

Preferred Experience

• Experience in home health care delivery or outpatient physical therapy settings.

• Familiarity with digital applications or solutions for health management and reporting.

• Prior involvement in team-based care coordination and inter-professional collaboration is a plus.

Compensation and Benefits
