Position Summary The Case Manager Ifor La Jornadais a child welfare professional responsible for the completion and submission oftimelyassessments and service plans, ensuring document uploads into ORR’s UC Portal andmaintainingrequired comprehensive case files compliant with ORR Policy and NYAP’s CQI Team. RESPONSIBILITIES The Case Manager Iwill perform duties including,but not limited to: Perform all work in a manner consistent with the National Youth Advocate Program’s Mission, Values andPhilosophies. Coordinate case management services for children in care under the direction of the Lead Case Manager. Maintain active caseload and work on sponsorship and reunification process. Ensure case management assessments are completed within ORR’s allottedtimeframes. Coordinatetimelyreferrals to external care providers andconsistentcommunication with relevant stakeholders on referral status, ensuring consistent carecoordination communicationbetween medical and behavioral health staff internally and externally. Coordinatetimelyreferrals to child advocate services as needed, ensuring that theappropriate healthinterpretation and translation services are available, and ensuring coordinator ofappropriatepost-releaseservices that include the continuity of health care services Conduct on-site admission and initial intake interviewsofyouth. Conduct interviewsofsponsors/family members,or workalongsidethe Providencia Group (TPG) to complete sponsor vetting. Participate inpre staffingand staffingprocess. Coordinate withassigned legal service providers (LSP)for thetimelyprovision of “Know Your Rights” presentations and legal screenings to children incare. Work to ensure children in care areprovideda safe environment and safe and timely release from ORR care. Provide weekly face to face updates to youth, telephoniccontactand updates to family members/sponsor; Effectively coach, mentorguidecase managers in ORR policy,procedureand child welfare best practices. Establish andmaintaina strong relationship with assigned foster parents, attend meetings with fosterparents,and act as program liaison with fostercaregivers. Drive children tofacilitateprogram services, may include transportation to court appointments, attorney visits, other appointments as needed per contractual duties; this may also include transporting youth withinor outsidethe United States for reunificationpurposes. Activelyparticipatein the process of reportingsignificant incidents (SIRs)in accordance withexisting policies and procedures. Ensure maintenance of UC electronic and physical files, including uploading documents in UC Portal, Evolve, andmaintainingthe corresponding physicalfile. Participate in the Continuous Quality Improvement activities on a weekly/monthly/quarterlybasis. Perform other duties as assigned. MINIMUM QUALIFICATIONS Bachelor’s degree in the behavioral sciences, humanservicesor social services fields. Child welfare and/or case management experience is strongly encouraged. Required toworka flexible schedule tofacilitateprogram services, including working on call schedules whichincludesweekends and evenings. Proficient use of desktop and laptop computers, smartphones and tablets, printers, fax machines, and photocopiers, as well as software including word processing,spreadsheet, and database programs. Bilingual (English/Spanish). Fluency in Spanish is required. Minimumautomobile insurance coverage of $100,000/300,000 bodily liability coverage. 21years of age,a validstate driver’s license,reliablepersonal vehicle, anda good drivingrecord. OTHER SKILLS Must be able towork8 UC cases with minimalsupervision butmay increase depending on sponsor category and other factors. Ensure the proper supervision of youth at all times. Demonstrates tolerance and respect for the ideas and actions of others. Possess a sincere desire and ability to advocate for children facing behavioral health, immigration, educational, legal, development, and socio-economic challenges. PHYSICAL DEMANDS Use of manual dexterity, tactile, visual, and audio acuity. Use of repetitive motion, prolonged periods of sitting and standing, and sustained visual and mental applications and demands. Occasional lifting (up to 25 pounds), bending, pulling, and carrying. Ability to travelfrequentlyand drivevehiclewhile sitting for extended periods, with frequency varying based on program demands.
