The Admissions Patient Coordinator serves as a key patient-facing role within the Admissions Department, supporting patients throughout the Initial Home Health Care admissions and authorization process. This position is responsible for coordinating physician appointments, nursing assessments, home visits, caregiver onboarding, and patient communication to ensure timely and effective initiation of services.
The Admissions Patient Coordinator acts as the primary point of contact for patients and families, providing education, support, advocacy, and ongoing communication throughout the process. Responsibilities include attending physician appointments, coordinating documentation and scheduling, facilitating communication between departments, assisting with onboarding and workflow progression, and ensuring patients receive a positive and supportive experience from initial approval through care initiation.
This role requires strong organization, communication, problem-solving, and time management skills, as well as the ability to work effectively in a fast-paced healthcare environment while maintaining a patient-centered approach to care. Candidates should be compassionate, adaptable, detail-oriented, and comfortable collaborating with patients, caregivers, healthcare providers, and interdisciplinary teams.
Responsibilities:
- Serve as the primary point of contact for patients throughout the Initial Home Health Care admissions and onboarding process, ensuring consistent communication, support, and advocacy.
- Manage an independent caseload of patients through the admissions, authorization, and onboarding process while maintaining workflow progression and timely follow-up.
- Coordinate physician appointments, nursing assessments, home visits, caregiver onboarding, and required documentation to support timely initiation of care services.
- Conduct home visits to establish patient relationships, provide education regarding the home health program, assess patient needs, gather documentation, and support onboarding activities.
- Attend and advocate for patients during physician appointments, facilitating communication between providers, patients, and internal departments while assisting with obtaining required signatures, documentation, and orders.
- Coordinate and communicate with patients, families, physician offices, Resource Nurses, Authorization Specialists, Writing, Staffing, and operational departments to support continuity of care and workflow progression.
- Assist with obtaining, uploading, routing, and tracking required documentation including visit notes, Plans of Care, Letters of Medical Necessity (LMNs), assessments, and physician documentation.
- Coordinate Durable Medical Equipment (DME) requests and communicate patient needs to appropriate departments while ensuring patients understand equipment usage and expectations.
- Provide courtesy, milestone, and follow-up calls to patients regarding approvals, onboarding status, scheduling updates, authorization progression, and care initiation.
- Maintain accurate documentation, workflow updates, provider information, and patient records within Salesforce, EMR systems, and internal company platforms within required timelines.
- Assist with caregiver onboarding coordination, including communication regarding onboarding requirements, credentialing items, scheduling, and readiness for patient care assignments.
- Identify barriers, workflow delays, patient concerns, or compliance issues and escalate concerns appropriately to leadership or supporting departments.
- Collaborate closely with Admissions Leads and interdisciplinary teams while maintaining the ability to work independently, prioritize tasks, and manage competing responsibilities in a fast-paced environment.
- Pull, review, and monitor workflow reports, patient progression data, and operational metrics to support departmental goals, accountability, and quality standards.
- Provide exceptional customer service while building compassionate, professional relationships with patients, families, caregivers, and healthcare providers.
- Support additional operational needs and assist team members as necessary to ensure patient needs and departmental objectives are met.
- Meet and exceed established productivity, quality, workflow, and patient satisfaction metrics.
Qualifications:
- 25 years of experience in healthcare coordination, admissions, patient navigation, scheduling, case management, customer service, or related healthcare roles preferred.
- Healthcare or home health experience preferred.
- High school diploma or equivalent required.
- Valid driver's license, current auto insurance, and dependable personal vehicle required for patient visits and local travel.
- Ability to travel locally for patient home visits, physician appointments, and operational support as needed.
- Strong ability to work independently with minimal supervision while managing a high-volume caseload and competing priorities.
- Computer literacy and comfort utilizing laptops, tablets, EMR systems, Salesforce, Microsoft Office Suite, and operational software platforms as primary daily tools.
- Strong written and verbal communication skills with the ability to communicate professionally and compassionately with patients, families, providers, and interdisciplinary teams.
- Excellent organizational skills, time management, attention to detail, problem-solving ability, and adaptability in a fast-paced environment.
- Understanding of HIPAA regulations and commitment to maintaining patient confidentiality and compliance standards.
- Flexible availability outside of normal business hours as patient needs require.
