Job Summary:

The Occupational Therapist (OT) provides evaluation and therapeutic services to children, adolescents, adults, and seniors with developmental and intellectual disabilities and related support needs, promoting functional independence, daily living skills, and meaningful participation in daily and program-related activities. This role delivers participant-centered, facility-based occupational therapy services in compliance with professional standards, regulatory requirements, and Harbor House policies.

Key Responsibilities :

Evaluation & Treatment:

• Conduct occupational therapy evaluations and assessments to identify participant strengths, needs, and goals in accordance with physician orders and professional standards.
• Develop, implement, and modify individualized plans of care addressing functional performance, sensory processing, fine and gross motor skills, self-care, cognitive functioning, and adaptive skills, as appropriate.
• Deliver therapeutic interventions that promote participant independence, safety, participation, and quality of life.
• Monitor participant progress toward established goals and adjust treatment approaches as clinically appropriate to ensure continuity and effectiveness of care.
• Administer standardized and functional assessments as required to support treatment planning and outcomes measurement.

Program Support & Therapeutic Activities:

• Design and implement therapy activities and interventions that are developmentally appropriate and aligned with best practices across adult and pediatric programs.
• Modify activities, environments, and routines to support functional independence and participant engagement.
• Support pre-vocational and functional skill development activities, as applicable to participant needs and program scope.

Education & Participant Support:

• Educate participants and families on therapy goals, treatment approaches, and strategies that support functional progress.
• Provide guidance on environmental modifications and daily living adaptations to enhance independence and safety.
• Instruct participants, families, and staff in the appropriate selection and use of adaptive equipment and assistive technology.

Documentation, Quality Assurance & Compliance:

• Complete all required clinical documentation accurately and timely, including evaluations, progress notes, treatment records, and discharge summaries.
• Maintain documentation in compliance with state, federal, payer, and Harbor House requirements.
• Monitor and evaluate participant progress and therapeutic outcomes to support quality improvement.
• Adhere to HIPAA, accreditation standards, infection control policies, and participant rights requirements.

Interdisciplinary Collaboration:

• Collaborate with the Therapy Manager and interdisciplinary team members to support coordinated care and consistent therapeutic approaches.
• Participate in care planning meetings, staffing meetings, and other interdisciplinary forums as required.
• Communicate effectively with participants, families, program staff, and external partners to support continuity of care.

COTA Collaboration & Support:

• Collaborate with Certified Occupational Therapy Assistants (COTAs) to support the effective implementation of occupational therapy treatment plans.
• Provide clinical guidance, direction, and feedback to COTAs in accordance with professional standards and scope of practice.
• Review and contribute to documentation completed by COTAs, as required, to ensure accuracy, continuity of care, and compliance with applicable regulations.
• Communicate treatment goals, progress, and modifications to COTAs to support consistent service delivery.

Qualifications & Skills:

• Graduate of an accredited Occupational Therapy program, meeting one (1) of the following:

Ø  Doctor of Occupational Therapy (OTD), or

Ø  Master’s degree in Occupational Therapy (MSOT/MOT).

·         Current, active Occupational Therapist (OT) license in the state of Kentucky.

·         CPR/BLS certification or ability to obtain within a specified timeframe.

·         Demonstrated ability to provide occupational therapy services to individuals with intellectual and/or developmental disabilities or comparable populations.

·         Strong clinical judgment, organizational skills, and documentation abilities.

·         Ability to work collaboratively within an interdisciplinary team environment.

·         Demonstrate professional, ethical, and respectful conduct in all interactions with participants, families, and staff.

Physical Requirements:

·         Ability to stand, walk, bend, stoop, reach, and move throughout the workday.

·         Ability to assist participants with positioning, movement, and therapeutic activities.

·         Ability to lift and move up to 50 pounds, with or without reasonable accommodation.

Now Harbor House is expanding to bring these same outstanding day services to children and seniors of all abilities—meaning with and without disabilities—on an intergenerational model where all three groups will engage daily in meaningful opportunities aimed at improving their lives. What does that mean? It means that children, adults, and seniors of all abilities will read, sing, dance, paint, draw, and even exercise together.

We moved into our new 36,000-square-foot Intergenerational Life Center in October 2023, and are now weaving children and seniors into our family, beginning with before- and after-school programs and seniors in 2024, and 105 children of all abilities, ages 6 weeks to 5 years, when our previous 8,000-square-foot building is renovated into a Child Enrichment Center by spring 2025.
