Role: RN Case Manager responsible for facilitating patient-centered, holistic, and collaborative care. Key Responsibilities Assess and plan patient care to ensure optimal treatment pathways. Manage seamless continuity of care for patients with diverse needs, from chronic to critical cases. Align patient requirements with appropriate medical services through comprehensive evaluation. Utilize evidence-based practices to develop and implement care plans. Qualifications & Skills Experience as an RN in case management or similar role. Strong assessment, planning, and coordination skills. Knowledge of healthcare protocols and evidence-based practices. Ability to collaborate effectively with multidisciplinary teams. #J-18808-Ljbffr
