A rewarding position awaits an experienced Registered Nurse eager to make a difference through in-home patient care. This full-time, 13-week contract is ideal for a clinically confident RN with proven OASIS and home health experience, ready to thrive in a structured environment supporting patients in their own residences.

Schedule:

• Full-time, Monday through Friday
• On-call probable

Key Responsibilities:

• Deliver skilled nursing care, clinical interventions, and patient education directly in patients homes
• Accurately complete and submit OASIS assessments and documentation on time
• Collaborate with physicians and interdisciplinary teams to create and implement patient care plans
• Perform wound care, manage medications, monitor chronic conditions, and assess disease progression
• Provide clear communication regarding patient condition changes with the clinical team
• Travel between patient locations, maintaining professionalism and timeliness

Desired Qualifications:

• Active RN license
• Minimum of one year recent home health RN experience
• OASIS expertise required
• Strong proficiency with HomeCare HomeBase (preferred)
• Capable of managing daily patient visits with attention to detail and documentation
• Reliable transportation and valid drivers license
