A day in the life

Youll advocate for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs. You will serve as a resource for healthcare professionals through fostering relationships within medical facilities. With clinical expertise and problem-solving skills youll also coordinate care plans with physicians and families. Additionally you will facilitate patient transfers and address barriers to accessing specialized post-ICU care.

Responsibilities

What a Clinical Liaison Does

Key Responsibilities:

• You will work with physicians discharge planners and patients families to plan the best continuum of care after an ICU or post-surgical stay by utilizing your strong clinical background payer knowledge and problem-solving skills.
• Perform clinical assessments of referred patients then counsel with patients families to assist them in making the best care decisions for their loved one.
• Partner with hospital case managers and physicians to create a smooth pre-admission and transfer process through removing payer-related and operational barriers for patients getting into our highly effective level of care.

Qualifications

Minimum Qualifications

• Current and valid state licensure or eligibility for state licensure as a Registered Nurse.

Preferred Experience

• Two years of related experience working in a clinical healthcare setting or previous experience working in a clinical liaison role are preferred.
• Demonstrated competency with the preadmission process including assessments and relationship building in a rehabilitation setting or in another post-acute care setting is preferred.
• Proven track record in sales and marketing with a least one year of experience in a related position preferred.
• Demonstrated computer proficiency.
• Must exercise sound judgment in handling the professional/confidential nature of health care sales.
