Overview Field Care Coordinator - Warsaw, VA and surrounding markets. Field-based role with a home-based office located in Warsaw, VA. This role supports enrollees by facilitating assessment, planning, and coordination of care with the interdisciplinary team, traveling to enrollees’ homes, nursing facilities,Adult Day Health, and Adult Living Facilities to conduct assessments and develop care plans. Responsibilities Engage with members face-to-face and/or by telephone to complete a comprehensive needs assessment across medical, behavioral, functional, cultural, and socioeconomic domains. Develop and implement person-centered care plans addressing chronic condition management, health promotion, social determinants of health, medication management, and safety in alignment with evidence-based guidelines. Collaborate with the internal care team, providers, and community resources to implement the care plan. Provide education and coaching to support member self-management of care needs and lifestyle changes. Support proactive discharge planning and coordinate care transitions after ER visits, hospital admissions, or Skilled Nursing Facility (SNF) stays. Advocate for members and families to ensure needs and choices are represented and supported by the health care team. Act as liaison between the health plan, the Commonwealth, enrollees, and their families; follow established standards of care and applicable guidelines and procedures. Required Qualifications Current and unrestricted Licensed Practical Nurse in the state of Virginia or Social Work / Human Services (or related field) with a 4-year degree. 3+ years of care coordination or behavioral health experience in a healthcare setting. 1+ year of experience with MS Office (Word, Excel, Outlook). Experience working with members who have medical needs, the elderly, individuals with physical disabilities, and/or those with communication barriers. Access to reliable transportation and a valid US driver’s license with the ability to travel within the assigned territory. Preferred Qualifications CCM certification Experience working with Medicaid/Medicare populations Experience in team-based care Long-term care / geriatric experience Background in Managed Care Physical Requirements Ability to travel between office and field locations multiple times per day Ability to navigate multiple locations/terrains to visit members, providers, and colleagues Ability to transport equipment (laptop, stethoscope, etc.) to field locations Ability to remain stationary for extended periods to complete computer or tablet work *All telecommuters must adhere to UnitedHealth Group’s Telecommuter Policy. Pay is based on factors including local labor markets, education, experience, and certifications. The role offers a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401k contribution, subject to eligibility. The hourly pay for this role ranges from $24 to $43 per hour based on full-time employment. We comply with all applicable minimum wage laws.
