RN Staff Nurse – Assertive Community Treatment (ACT) TeamThe purpose of the RN Staff Nurse – Assertive Community Treatment (ACT) Team role is to provide comprehensive nursing care and clinical support to individuals with severe and persistent mental illness and co-occurring conditions within a community-based treatment setting. This role is responsible for managing and administering medications, conducting clinical and psychiatric assessments, addressing physical health needs, and coordinating care with medical providers and the interdisciplinary ACT team. The program employs master's and bachelor's-level counselors, a registered nurse, a substance abuse specialist, a vocational specialist, a psychiatrist/APN, a peer support specialist, and program assistant. Team members work together with clients to promote symptom stability and appropriate use of medication; restore personal, community living, and social skills; promote and maintain physical health; establish access to entitlements, housing, work, and social opportunities; and promote and maintain the highest possible level of functioning in the community. This program is funded by and part of the system of public services offered by Delaware DSAMH.
