Vocational Specialist-ACT Team

Founded in 1974, Community Access continues to lead supportive housing and social services in NYC for individuals with mental health concerns, promoting human rights, social justice, and economic opportunity across 25 locations. Program/Department Description Assertive Community Treatment (ACT) is an evidence-based mobile practice model that provides treatment, rehabilitation, and support services to individuals with serious mental health conditions whose needs have not been met by traditional services. The Bronx Shelter ACT Team serves homeless individuals with mental health conditions in the Bronx and other boroughs, collaborating with shelter staff and using assertive engagement strategies to help participants achieve housing and personal goals. Overview The ACT Team Vocational Specialist meets participants in community settings, providing holistic recovery services and ongoing supportive counseling. The Specialist coordinates services for homeless adults in Bronx mental health shelters and those recently transitioned to supportive or independent housing, integrating person-centered goals with the tasks of all ACT team members and delivering psycho-education individually and in groups. The role supports vocational development, helping participants identify schools, training, jobs, or employment programs that align with their goals. Key Performance Indicators Complete assigned assessments and service plans on time. Submit progress notes for all contacts, collaterals, and attempts within two business days. Job Qualifications Demonstrated understanding, appreciation, and commitment to the philosophy and mission of Community Access. Commitment to person-centered treatment strategies, upholding participants’ rights and self-determination. Licensed Master of Social Work, Licensed Mental Health Counselor, or a master's degree in a related field. Knowledge of New York City/New York State vocational programs and services. Knowledge of entitlement systems. Ability to prioritize and meet deadlines. Creativity and flexibility. Initiative and follow-through responsibility. Capability to work as part of a multi-disciplinary team. Strong conflict mediation/negotiation skills and an assertive problem-solving approach. Excellent oral, written, and listening skills. Computer proficiency, including Microsoft Word and Excel; comfort with new systems and paperless record keeping. Spanish speaking preferred. Fingerprinting and clearance by the New York State Justice Center required. Ability to use public transportation throughout the city for assigned tasks.
